MECHANICAL INSTALLATION

§ Customer will need to unload and position the unit near final placement

§ Installation is assumed to be conducted in one continuous phase.

§ Pricing does not include additional mobilizations.

§ Additional work performed beyond the scope of the project will be invoiced at the enclosed labor and material rates. Written approval will be obtained before the installer performs any work that is considered “additional” to the written contract.

§ Installation is expected to be Monday-Friday @ 8-10 hours per day.

§ Midland Packaging will not be held responsible or incur additional cost for any delay or late startup due to: Acts of God, strikes, accidents, force majeure, or delays caused by others.

GENERAL ASSUMPTIONS

Customer to Supply:

* Normal utilities 110v / 220v / 480v for install.

* If Normal utilities are not available, we require 100/60A-480V power feed connection to power our portable breaker panel/transformer for our own power use,

* Sanitary and Break room facilities,

* A clear unobstructed path for all equipment including staging space,

* Adequate lighting and heating,

* Reasonable and adequate work and lay-down area,

* Disposal container and all disposal cost,

* Bathroom facilities for installation personnel

* All Permits Fees as needed.

* 480VAC/240V and 120V power feeders run and terminated to the Midland Packaging supplied control panels, this would include breakers or bus-plugs for power feeders within the source,

* 120VAC convenience outlets as required for all workstation, printers, etc.,

* Customer to supply air at system or to our main control valves from the customer supplied compressor.

* No Fire protection included

* All Freight and Taxes are excluded from the Midland pricing summary.

Approval drawings required

* Customer approval of drawings required prior to start of manufacturing.

General notes

* Unless otherwise stated, all equipment, tolerances, finishes, drawings and documentation are quoted to Conveyor Manufacturer standards.

Customer electrical responsibility

* Customer is responsible for the following electrical requirements: conforming to local and national electrical codes and regulations, field wiring, any program modifications, short-circuit protection for all motor and control circuits, safety devices, interlock programming, and any other additional electrical devices required at installation to meet the application requirements.

Customer safety responsibility

* Safety device and procedure requirements vary with each application of this equipment. Compliance with all federal, state, OSHA and local laws or codes is the responsibility of THE CUSTOMER.

No special environment provisions

* No provisions have been made in this quote for any special environmental or working conditions that may exist such as; temperature, dust, dirt, wet, chemical, acidic, caustic, or explosive.

Product contact surfaces and structural integrity

* It is THE CUSTOMER’s responsibility to determine if the product to be conveyed possesses the structural integrity and contact surfaces necessary to convey satisfactorily on the equipment quoted and through transition areas.

Building Assumptions

* All equipment listed within this proposal will be installed in a clear and unobstructed area. This area should be free of a pre-hung drop ceiling, lighting fixtures or other building obstructions. The floor and ceiling construction is adequate to support the load imposed and movement of the equipment.

* No special shoring or seismic bracing.

Mechanical Provisions by Midland

* All Conveyor, Conveyor Controls, Mechanical Conveyor & Controls Installation, commissioning,

* Air piping or air install within system only –

* Installation of equipment not referred to in the project scope or illustrated and itemized on the layout drawing. (Safety Barriers or Highway type guarding not included)

Testing Requirements

* Personnel to operate the system during system testing, along with product handlers to move test product,

* Adequate test product (cartons, totes) representing min, max, and average product mix to perform system acceptance testing,

* On-site Integration support for system testing other than stated in this proposal,

* Any item or service not specifically provided by Midland Packaging.

DESCRIPTION OF ORDER STARTUP

Prerequisites for start-up are:

* Acceptance of order confirmation/assignment description

* Confirmation of start-up date

Midland must be notified of any changes to the assignment before the work is performed or as soon as possible before delivery takes place. Changes must be documented in writing. Any accepted changes will be regarded as a ‘Change Request’ and will be settled separately in relation to this document.

PROJECT MANAGEMENT

Midland will provide engineering and project management to incorporate all the engineering, planning, task scheduling, coordination and on-site installation monitoring, testing and training of all activities and resources, including internal THE CUSTOMER staff, technology partners and installation groups.

Specific responsibilities and tasks will include the following:

* Develop a detailed project plan with activities and resource assignments

* Communicate and coordinate with all project resources as they are required to perform and participate in each stage of the project

* Provide status reports as required

* Oversee all installation activities coordinating with THE CUSTOMER operations

* Test and commission systems components through end-to-end acceptance

* Develop a Go-Live Plan and activities for maximum project visibility

Midland will assign a project resource during the project from order entry thru go-live. At times we may bring in additional resources to support specific tasks.
